Biological Sample Use Request

In order to accept biological samples to the ACU Biobank Unit, the ACUTAB-Biobank Unit Biological Sample Storage Request Form must be filled out and sent via e-mail to acutab@acibadem.edu.tr. The registered request is then forwarded to the ACU Biobank Unit and subjected to preliminary evaluation by the Administrative Board; acceptance or rejection information regarding the biological sample/data storage request is sent to the applicant within 3 business days. The ACU Biobank Agreement, ACU Biobank Biological Sample Submission Form and ACU_Biobank Informed_Voluntary Consent_Form are sent via e-mail to researchers whose biological sample/data storage application preliminary request is positive. Afterwards, the sample(s) prepared in accordance with the biological material acceptance conditions specified below should be delivered to the ACU Biobank Unit together with the printouts of the ACU Biobank Agreement and ACU Biobank Biological Sample Submission Form with wet signature.
